Sol alt kadran ağrısının ender bir sebebi akut apandisit: Olgu sunumu

Abstract

Sol alt kadran ağrısının birçok sebebi mevcuttur, bunlardan biri de genellikle intestinal malrotasyon (İM) ile birlikte görülen ve solda yerleşen apendiksin akut inflamasyonudur. İM orta bağırsak (midgut) bölümünün peritoneal kavitede arteria mesenterica superior etrafında normal fetal rotasyonunu yapamaması ve fiksasyon bozukluğu ile seyreden bir gelişimsel anomalidir. Bu yazımızda sol alt kadran ağrısı ile tetkik edilen ve tanı koymada mevcut radyolojik yöntemlere rağmen zorluk çekilen ve sonuçta tanısal laparoskopi uygulanan bir vaka sunuldu. Geç tanı neticesinde morbitite ve mortalite artmaktadır. Komplikasyonların önlenmesi için sol alt kadran ağrılarında akut apandisit akılda tutulmalı, ileri tetkikler yapılmalı ve gereğinde tanısal laparoskopiden kaçınılmamalıdır

There are many causes of pain in the left lower quadrant, one of them is appendicitis which is due to acute inflammation of leftly placed appendix that mostly associated with intestinal malrotation (İM). İM is developmental anomaly, which is accompanied by unable to normal fetal rotation of midgut portion in the peritoneal cavity around the superior mesenteric artery and fixation disorder. In this paper a case is presented that examine with lower left quadrant pain and diffucult to diagnose despite the current radiological methods and ultimately laparoscopy was applied. As a result of late diagnosis morbidity and mortality increas. For the prevention of the complications in the lower left quadrant pain, acute appendicitis should be kept in mind, further investigations should be made, and when necessary, diagnostic laparoscopy should not be avoided.
