Physical function and performance effects of platelet-rich plasma (PRP) in early-stage knee osteoarthritis

dc.description.abstractAim: Osteoarthritis (OA) is a common, painful and degenerative joint disease that causes functional limitation. Pharmaceutical agents, exercise, physical therapy, intra-articular injections and surgery can be counted among treatment applications. In recent years, platelet-rich plasma (PRP) has been considered as an autologous biological therapy used to stimulate cartilage healing and improve symptoms. Although the effects of PRP injection on pain and function of knee OA patients have been widely investigated before, there have been very limited studies about the effects on physical function and performance. In this study, it was aimed to investigate the effectiveness of PRP in improving physical function and performance in patients with knee OA. Material and Methods: This study was performed with 80 stage 1-2 bilateral knee OA patients according to the Kellgren-Lawrence Scale. Intra-articular PRP injection was applied by the same physician at one month intervals. Physical performances of the patients were analyzed twice, just before the first injection and six months after the last injection, with Stair Climb Test (SCT), 50-Foot Walk Test (50 FWT), 30-second Chair Stand Test (30 CST) and Timed Up and Go Test (TUG). In addition, Hospital for Special Surgery (HSS) knee scoring was also recorded as secondary functional result criteria. Paired-t test was used to assess the significance of the difference in before and after injection results. Results: Statistically significant improvement was observed in all physical performance tests (SCT, 50 FWT, 30 CST, TUG) and functional scoring (HSS) at 6 months after PRP application (p<0.001). Conclusion: PRP is a safe and effective autologous biological agent in improving physical function and performance in patients with mild to moderate knee OA.
dc.description.abstractAmaç: Osteoartrit (OA), fonksiyonel kısıtlamaya neden olan yaygın, ağrılı ve dejeneratif bir eklem hastalığıdır. Tedavi uygulamaları arasında farmakolojik ajanlar, egzersiz, fizik tedavi, eklem içi enjeksiyonlar ve cerrahi sayılabilir. Son yıllarda, trombosit açısından zengin plazma (PRP), kıkırdak iyileşmesini uyarmak ve semptomları iyileştirmek için kullanılan otolog bir biyolojik tedavi olarak kabul edilmektedir. PRP enjeksiyonunun diz OA hastalarının ağrı ve fonksiyonu üzerindeki etkileri daha önce geniş çapta araştırılmış olsa da, fiziksel fonksiyon ve performans üzerindeki etkileri hakkında çok sınırlı çalışma yapılmıştır. Bu çalışmada, diz OA hastalarında fiziksel fonksiyon ve performansı iyileştirmede PRP'nin etkinliğini araştırmak amaçlanmıştır. Materyal ve Yöntemler: Bu çalışma, Kellgren-Lawrence Ölçeğine göre evre 1-2 bilateral diz OA'lı 80 hasta ile gerçekleştirilmiştir. Eklem içi PRP enjeksiyonu, aynı hekim tarafından birer aylık aralıklarla uygulanmıştır. Hastaların fiziksel performansları ilk enjeksiyondan hemen önce ve son enjeksiyondan altı ay sonra olmak üzere iki kez Merdiven Tırmanma Testi (SCT), 50 Ayak Yürüme Testi (50 FWT), 30 Saniyelik Sandalyeden Kalkma Testi (30 CST) ve Zamanlı Kalkma ve Yürüme Testi (TUG) ile analiz edildi. Ek olarak, Özel Cerrahi Hastanesi (HSS) diz skorlaması da ikincil fonksiyonel sonuç kriterleri olarak kaydedildi. Enjeksiyon öncesi ve sonrası sonuçlardaki farkın anlamlılığını değerlendirmek için eşleştirilmiş t testi kullanıldı. Sonuçlar: PRP uygulamasından 6 ay sonra tüm fiziksel performans testlerinde (SCT, 50 FWT, 30 CST, TUG) ve fonksiyonel skorlamada (HSS) istatistiksel olarak anlamlı iyileşme gözlendi (p<0.001). Sonuç: PRP, hafif ila orta derecede diz OA'sı olan hastalarda fiziksel fonksiyonu ve performansı iyileştirmede güvenli ve etkili bir otolog biyolojik ajandır.
